Plot summary
Henry Porter has been in group therapy at a mental health facility and is well enough to look for work, so his sister Laura takes his place in the group. Their parents were killed by a hooded man when the two were children, and both believe the Boogeyman did it. The other members include Mark, who fears the dark, Paul, who fears germs, Alison, who harms herself, Darren, who is afraid of open places, and Nicky, who fears gaining weight. Dr. Jessica Ryan works at the facility, and Dr. Mitchell Allen has treated patients there. Not long after Laura arrives, the members are attacked one by one in ways that match their fears.

Can contain spoilers, watch the movie first.
- Tobin Bell, who played therapist Dr. Mitchell Allen in Boogeyman 2, modeled his portrayal of the character after then-US Vice President Dick Cheney.
- The Boogeyman's mask was designed by Jerad S. Marantz, and the creature's look was based on director Jeff Betancourt's own childhood fears, with "skeletal things and bird corpses" inspiring its skin texture.
- Danielle Savre, who played lead Laura Porter, spent the shoot running up and down hospital corridors screaming while director Jeff Betancourt's crew threw blood, vomit and guts on her.
- With a low budget that let most effects be filmed only once, extensive storyboards were used to plan the shoot and keep make-up artists from wasting effort on unused prosthetics.
- Renee O'Connor got the part of Dr. Jessica Ryan by emailing producer Rob Tapert, with whom she had a contract, and telling him she really wanted to play the character.
