Plot summary
On Lindisfarne, an island joined to the mainland by a causeway that the tide covers, a community of survivors subsists, decades after the Rage virus broke out. Jamie works as a scavenger; his wife Isla has undiagnosed cancer; their son Spike is twelve. Early in the outbreak, a boy named Jimmy Crystal fled his home in the Scottish Highlands and took shelter in a church with his father. The virus was eradicated from continental Europe, but the British Isles remain under indefinite quarantine. Jamie and Spike cross to the mainland for a coming-of-age hunting ritual, and Spike kills an Infected there. A pack led by an evolved Alpha, stronger and more intelligent than the rest, hunts them.

- The actors playing the nude Infected were required by law to cover their own genitalia with prosthetic genitalia, according to Danny Boyle, because child actor Alfie Williams was present on the set.
- Alex Garland first wrote a spec script titled 28 Months Later in which the rage virus is weaponised and Chinese special forces hunt the original lab for a vaccine, with the whole film in subtitled Mandarin; Danny Boyle rejected it as an overused horror trope.
- The film was shot primarily on an iPhone 15 Pro Max alongside action cameras and drones, echoing the Canon XL-1 camcorder used on the original 28 Days Later for its manoeuvrability in deserted locales.
- Some shots used a bullet time effect made with a custom-built circular rig holding between eight and twenty iPhone cameras, which also let Danny Boyle film scenes from several angles at once for more editing flexibility.
- The Sycamore Gap tree, felled by vandals in 2023, was digitally reconstructed for the film because the vandalism is presumed never to have happened in its alternate timeline.
- Cillian Murphy was initially reported to be returning as Jim, but producer Andrew Macdonald confirmed in January 2025 that he does not appear, though he stayed on as an executive producer.
