Plot summary
Casey and Miller want to take their college friend Jeff Chang out for his twenty-first birthday, but Jeff has a medical school interview the next morning that his father arranged, and he agrees to only one drink. At the bar Jeff accidentally hits a stranger with a dart, and the three of them flee, cutting short Casey's conversation with Jeff's friend Nicole. Jeff drinks far more than he meant to and passes out. Casey and Miller mean to take him home but cannot remember where he lives. Believing Nicole knows the address, they climb into what they take to be her sorority house.

- Relativity Media drew criticism from civil rights advocates for shooting part of 21 & Over in Linyi, Shandong province, China, ignoring the area's human rights record, notably the house arrest of activist lawyer Chen Guangcheng in a nearby village.
- For the Chinese release, directors Jon Lucas and Scott Moore added scenes at the start and end of 21 & Over and changed the lead to a Chinese student who briefly transfers to an American college; Lucas said the version is about a boy corrupted by Western partying who goes back to China a better person.
