Plot summary
Alison Parker is a fashion model who, as a young girl, attempted suicide after her father's sexual behavior traumatized her. She rents an apartment in a Brooklyn Heights brownstone where a blind priest occupies the top floor, sitting at an open window. Soon she faints, cannot sleep, and hears noises. Her neighbors include an elderly eccentric named Charles Chazen and two women, Gerde Engstrom and Sandra, and she attends a strange birthday party for Chazen's cat. When Alison tells the rental agent, Miss Logan, about these people, Miss Logan says that only the priest and Alison are listed as living in the building. Alison's boyfriend Michael Lerman asks a detective he knows to look into the matter.

- For the film's Hell finale, director Michael Winner cast many of the deformed figures from hospitals and sideshows, revealing after release that they were actually people with physical disabilities and abnormalities.
- Universal rejected Martin Sheen as Alison Parker's attorney boyfriend Michael Lerman, feeling he had done too much television and lacked film appeal, so Michael Winner cast Chris Sarandon instead, after admiring him in Dog Day Afternoon.
- Christopher Walken took a minor supporting role as Detective Rizzo, and the film marked Beverly D'Angelo's first feature screen appearance.
- John Williams was originally hired to compose the score of The Sentinel, but he was replaced on the music by Gil Mellé.
