Plot summary
Nightmares have troubled Dennis since he was a child. One of the dreams shows the Cadaver, a demon that fixes a zipper to his back and enters his body. In the hidden room in the cellar he finds a mirror that he believes the Cadaver came from, and the reflection he speaks to seems to answer. Dennis's brother John treats caring for him as his own responsibility, while John's girlfriend Lydia wants to marry and is angry that John does nothing about Dennis. Mildy Torres, from social services, thinks Dennis should be taken away. The reflection tells Dennis he must make animal sacrifices to be rid of the Cadaver, and Dennis works in the secret cellar, where John sees his videos on taxidermy and embalming.

Can contain spoilers, watch the movie first.
- Getty kept working on The Evil Within for years after shooting, creating his own special effects, and died in 2015 with the coloring and editing not finished. Producer and editor Michael Luceri completed the film.
- Production on The Evil Within stopped and started over many years, set back by funding problems, conflicts with the cast and a lawsuit from a studio assistant. Actor Frederick Koehler said he and Michael Berryman were the only cast members to last through the changes.
- Getty said he was inspired by the David Berkowitz Son of Sam killings, in which Berkowitz claimed a demon in the form of a talking dog ordered him to kill, and asked what if a demon really was giving a serial killer orders.
- Getty converted a room in his own mansion into a post-production suite, made his own camera rigs, built expensive sets and, with his effects team, created elaborate animatronic robots, including an octopus that could play a drum kit.
- The Evil Within was the last film to feature Matthew McGrory, who appears in an uncredited cameo taken from footage shot before his death in 2005. Illness affecting McGrory and Michael Berryman had slowed production and forced scripted scenes to be replaced.
