Plot summary
Dr. Elizabeth Derby works as a psychiatrist and lives with her husband, Eddie. A young man named Asa Waite comes to her office and says that his father, Ephraim, sometimes uses his body. Ephraim telephones during the session, and Asa has a seizure before acting differently. That evening Elizabeth pictures Asa in Eddie's place. She goes to Asa's house and finds Ephraim gravely ill and kept alive by pills. When she notices a book filled with drawings of creatures and symbols, Ephraim wards her off with a large knife.

- Dennis Paoli had written a script based on Lovecraft's "The Thing on the Doorstep" years before Barbara Crampton asked him about scripts, the two having reconnected while arranging tributes to director Stuart Gordon, who died in 2020.
- Director Joe Lynch reworked the script by swapping the gender of the main character, and wanted the film to resemble the erotic thrillers of the late 1980s and early 1990s "because that's what the character would have found sexy."
