Plot summary
Marty DiBergi has had no Hollywood work since his documentary about Spinal Tap, so he follows the band once more. Hope Faith, daughter of the group's late manager Ian Faith, has inherited a contract that obliges the musicians to play one more concert. Marty tracks down Nigel Tufnel, who sells cheese and guitars in the north of England and plays in a local folk band; David St. Hubbins, who writes music for crime podcasts and telephone hold lines; and Derek Smalls, curator of a glue museum in London. Their last show was 15 years ago, and interest in the group has grown again. The three agree to perform, though tension sits between Nigel and David and a drummer must be found, since every earlier one died.

- A long legal fight over the original This Is Spinal Tap followed its 1984 release: in 2016 Harry Shearer sued Vivendi and StudioCanal, saying decades of merchandising had paid him only $81 and music sales $98, and sought up to $400 million.
- Live concert footage filmed at Stonehenge in August 2025 went unused; it was meant to become the 2026 cinema concert Spinal Tap at Stonehenge: The Final Finale, but the project was shelved after Rob Reiner and his wife were killed in December 2025.
- Rob Reiner had deliberately avoided making sequels, but the unexpected resurgence of Kate Bush through Stranger Things, plus continuing fan interest and the cast's frustration at earning little or no profit from the original film, changed his mind.
- The 2021 death of Tony Hendra, who played Spinal Tap's original manager Ian Faith, inspired the storyline in which his character wills a contract to his daughter Hope Faith requiring one last concert, forcing the band to reunite after 15 years.
- The film was only loosely scripted and largely improvised by the actors, in two to three takes per scene, with scenes captured using a dual digital camera setup so they could unfold spontaneously.
