Plot summary
Charlie MacKenzie performs beat poetry in San Francisco coffeehouses, drawing on the relationships he keeps ending. His friend Tony, a police detective, and his mother May both tell him the problem is his own paranoia. Shopping at a butcher shop for his parents' dinner, Charlie meets Harriet Michaels and is drawn to her. The two start dating, and Harriet makes a strong impression when Charlie brings her to meet his family. May reads sensational tabloids and has been following reports of a bride nicknamed Mrs. X, said to kill her husbands on their honeymoons with an axe. The tabloid lists a martial arts expert among the victims, and also a man named Ralph. Charlie has heard Harriet mention a martial arts trainer and cry out for Ralph while asleep. He asks Tony to investigate.

- While filming scenes in the butcher shop, Nancy Travis was distracted by Myers' antics and severed a fingertip on her left hand while chopping vegetables; a local doctor successfully re-attached the tip.
- Sharon Stone was initially set to play Harriet Michaels, the supposed axe murderer of the title, and wanted to play her sister Rose as well, but studio executives at Sony did not like the idea, so Stone refused the part and Rose went to Amanda Plummer.
- Before co-producer Cary Woods asked Mike Myers to play Charlie MacKenzie on the strength of Wayne's World, Woody Allen had considered the part, and Chevy Chase, Albert Brooks and Martin Short all looked at the role but did not like the character.
- To fit both Charlie and his father Stuart into the same scene, the filmmakers used a split-screen process, and playing Stuart required Myers to sit through three to four hours of prosthetic makeup.
- One scene on Alcatraz was filmed in “A” block, an area unchanged from its 1912 configuration and not open to the public.
