Plot summary
At a psychiatric ward in Newark, New Jersey, therapist Rose Cotter takes on a new patient, Laura Weaver, a PhD student who says an entity only she can see takes the form of smiling, menacing people and has foretold her death. Laura begins screaming and choking, then calms, smiles, and cuts her own throat in front of Rose. That night Rose sees a smiling Laura standing in her kitchen. More visions follow: a patient named Carl wears the same smile and shouts that she is going to die, yet Carl has been asleep throughout. Rose's supervisor orders her to take a week off. Rose's sister Holly and her fiancé Trevor believe her mother's mental illness has passed to her.
