Plot summary
In 1906, widower Henry McCarty and his son Wyatt live on a farm in the Oklahoma Territory, near the farm of Henry's brother-in-law, Al. A lost horse with a bloodstained saddle appears on their land, and Henry finds its owner, Curry, near death, with a pistol and a satchel of money beside him. Henry rides away intending to leave him, then returns and brings the unconscious Curry and the money into the farm. Curry says he is a sheriff, the only survivor of a posse sent after outlaws, and asks for help keeping them from the stolen money.

- Tim Blake Nelson feared the public would negatively compare his role as Henry in Old Henry to his performance as Buster Scruggs, characters he calls opposites. He and Potsy Ponciroli agreed the film would not feature gunspinning, and the final product keeps one spin in the finale as punctuation.
- Tim Blake Nelson said starring in the 2018 Western film The Ballad of Buster Scruggs taught him how to handle a gun. He worked with guns every day for about five months to be able to do the pistol tricks.
