Plot summary
Evelyn has come home to the Mountaintop Motel, the rural Louisiana cabins she rented out before she was committed to a psychiatric institution. Her daughter Lorie had died in the basement of the family home, and Evelyn convinced the authorities it was an accident, though suspicion stayed with her. Travelers settle in for the night: Reverend Bill McWiley, then Robin Crewshaw, who talk about how rundown the cabins are and share a drink; newlyweds Vernon and Mary, driving through on a road trip; and cousins Prissy and Tanya, bound for Nashville until their car breaks down, along with Al, the man who gives them a lift and says he is a record producer. A rainstorm makes the highway unsafe, so the guests stay.

When New World Pictures picked up the film in 1985, executive Roger Corman ordered reshoots because he felt Evelyn's death scene was not violent enough, so a section of the tunnel set was rebuilt and her killing was reworked into a near decapitation by a collapsing beam.- The Mountaintop Motel of the title was played by a dilapidated, abandoned fishing camp near Cross Lake in Shreveport, Louisiana, not a built set.
- The underground tunnel scenes were filmed above ground in makeshift sets draped in visqueen to keep light out, and the cabin interiors had to be repainted and redressed before shooting.
