Plot summary
Matt Leland is a high school senior in Seattle who looks after his nine-year-old twin siblings while his father works. Casey Roberts moves from Chicago into the house across the lake and jet skis at night, and Matt watches her through a telescope. She is a new student at his school, and he asks her out. Casey is furious when she learns about the telescope, though she forgives him after he says sorry, and the two begin a romance. She confides that her feelings shift between intense passion and fear and destructiveness. During Matt's SATs, Casey sets off the fire alarm and is suspended; her domineering father plans to send her to boarding school. That night she takes an overdose of sleeping pills and is placed in a psychiatric ward, where Matt visits her against his father's wishes.

- In the audio commentary for the film's Blu-ray release, journalist Bryan Reesman said that because of controversy over director Antonia Bird's 1994 film Priest, released while Mad Love was in production, studios cut Paula Milne's script and Bird's final cut, removing darker material about mental illness and teen suicide.
- Producer David Manson said the film's idea of a boy falling in love with a girl who has complicated emotional problems came out of his own experiences as an adolescent and a young man coming to terms with loss.
