Plot summary
A ship carries three passengers to Blood Island. Bill Foster is an American pathologist looking into a chlorophyll illness among the islanders; Sheila Willard has come to find her father; Carlos Lopez wants his mother, Mrs. Lopez, to leave. The captain calls the island cursed and tells of a man taken from a raft who bled green blood. Mrs. Lopez refuses to go, though her husband, Don Ramon Lopez, died on the island not long before. Dr. Lorca will say nothing about that death. The islanders talk of a green-skinned creature with chlorophyll blood that kills local natives. One night a native whose skin is covered in green sores tries to enter the government house and flees when he is seen.

- For The Mad Doctor of Blood Island, each theatergoer was given a free packet of colored liquid labeled "green blood" as part of a prologue, and at a cue the audience was told to recite the oath of green blood and drink it.
- In another gimmick, the camera on The Mad Doctor of Blood Island zoomed in and out rapidly whenever the monster killed or stalked someone; some theatergoers complained it made them dizzy, and it was designed to hide the shoddy make-up effects.
- Sam Sherman came up with the idea of handing the liquid gel-packs to theatergoers, and said he drank one, an aqua-colored gel, and it made him sick to his stomach, while associate producer Beverly Miller said he watched dozens of teenagers drink them in his Kansas City theaters.
- In 1969 some local newspapers, particularly in Rhode Island, began omitting the word "blood" from film titles they advertised, so The Mad Doctor of Blood Island ran as The Mad Doctor of Crimson Island and The Blood Demon became The Crimson Demon.
- John Ashley had starred in Brides of Blood, and it was popular enough in the United States that American distributors asked him to appear in a follow-up, which led to his moving to the Philippines and co-producing films with Eddie Romero, Beverly Miller and Roger Corman.
