Plot summary
Left for dead after being shot down a mine shaft, Michael Myers crawls from the river and is taken in by an elderly hermit living beside the water. Almost a year later he rises, puts on his mask and turns back toward Haddonfield, Illinois, and his niece, Jamie. Jamie has been confined to the Haddonfield Children's Clinic since his last attack and can no longer speak. She shares a psychic link with her uncle and falls into convulsions when he draws near, a connection Dr. Loomis hopes to use against him. Her foster sister Rachel visits with her friend Tina, and Dr. Loomis tells Rachel about a note found in town demanding that the evil child die.

- While making Halloween 5, Donald Pleasence accidentally broke Don Shanks' nose during the scene where Dr. Loomis beats The Shape with a block of wood.
- Director Dominique Othenin-Girard threw Shem Bitterman's draft into a trashcan in front of Moustapha Akkad, then brought in Robert Harders, of Home Movies, to write a new version in which Myers is resurrected but no longer evil and is hunted by an angry mob.
- Filming the scene where The Shape crashes Mike's Camaro into a tree, Don Shanks was injured when Othenin-Girard forgot to yell "Cut!" and fire began to emerge from the car; Wendy Kaplan was hurt when the car nearly ran over her.
- Halloween 5 was originally rated X by the MPAA for graphic violence, particularly the scene where Jamie is stabbed in the leg while hiding in a laundry chute, and a finale sequence in which Michael massacres a SWAT team at the Haddonfield hospital was cut in post-production.
- The film's opening hermit was first played by Theron Read as a character called "Dr. Death", with a shack full of runes, tablets and occult items used for necromancy, but the sequence was re-shot with an older actor and the occult material removed, and the Read footage was thought lost for years.

Cornell was unhappy that her character Rachel was killed off early, and in the original screenplay Rachel died after Michael shoved a pair of scissors down her throat; Cornell found that too gruesome and asked the writers to change it, so Rachel is stabbed in the chest instead.- George P. Wilbur, who had played Michael Myers in the previous film, did not want to return for Halloween 5 but worked as a stunt player; Don Shanks was cast as Myers instead and also played the mysterious Man in Black.
- The mask in Halloween 5 was designed by Greg Nicotero and molded from his own face, then widened to fit Don Shanks, whose head was larger than Nicotero's; Nicotero also oversaw the film's special effects with his KNB EFX Group.
