Plot summary
Detective James Wood is looking into a prescription pill mill operated by Dr. Kermit Gosnell when he finds out that a woman, Karnamaya Mongar, died at the clinic when staff gave her narcotics while no doctor was on the premises. Wood treats her death as a homicide and argues she deserves justice. With his partner, Detective Stark, he joins Drug Enforcement Administration and Federal Bureau of Investigation agents in a raid on the clinic. The officers find patients drugged ahead of abortions, cats wandering and defecating, and no sign of Gosnell. Wood brings in Assistant District Attorney Lexy McGuire, who takes up the prosecution of the physician.

- Filmmakers raising money for the film by crowdfunding said Kickstarter censored their project, asking that phrases like "1000s of babies stabbed to death" and "1000s of babies murdered" be removed or modified to fit its community guidelines. Kickstarter's CEO denied censoring and called it an editorial exchange blown out of proportion.
- The filmmakers moved to Indiegogo, launching their campaign on March 28, 2014 with a $2.1 million goal that was met less than two weeks later, on April 9, 2014. It quickly became the highest-funded film on Indiegogo and one of the highest-funded crowdfunding projects of all time.
- Days before a distribution deal was to be announced in June 2017, Philadelphia judge Jeffrey Minehart, who presided over Gosnell's trial, filed a $50,000 defamation lawsuit against the filmmakers, objecting to his portrayal. It was settled out of court and the filmmakers resumed negotiations with distributors.
- The producers tried to buy promotional airtime on National Public Radio, but NPR said the announcements could not use the terms "abortionist" or "abortion doctor." The producers refused the stipulation and did not go through with the purchase.
