Plot summary
During the German occupation of Holland, the SS lieutenant Kessel forces Father Lankester Merrin, a parish priest, to take part in executions after a trooper's murder. Merrin's faith is shattered. Three years later he works as an archaeologist at Derati, in British Kenya, digging at a Byzantine church built around the fifth century. Father Francis arrives at the dig. Chuma is Merrin's translator and guide, and Rachel Lesno, a doctor, and Emekwi, a convert, live at the site. The church is in perfect condition, as if buried at once. Merrin brings Cheche, a disabled young outcast, to Rachel for care. Inside the church a hidden passageway leads to a crypt holding a demonic idol and an ancient sanctuary of human sacrifice. Merrin concludes the church was built to seal that temple, and local elders demand the digging stop.

- After Exorcist: The Beginning, made for upward of $90 million, flopped in August 2004, Paul Schrader saw it with William Peter Blatty and told The Independent he bet there was a chance his own version could be resurrected.
- Morgan Creek fired Paul Schrader and scrapped his film entirely in August 2003, then hired Renny Harlin, one of the directors who met producer James G. Robinson and pitched rewriting the script, casting new actors and adding more action.
- In October 2001, Liam Neeson was attached to play Father Lankester Merrin in the prequel, which John Frankenheimer was set to direct from a screenplay revised by Caleb Carr.
- Angelo Badalamenti composed fourteen minutes of the score without payment, American metal band Dog Fashion Disco scored the final twenty minutes because of Schrader's son's fondness for the group, and about an hour was recycled from Trevor Rabin's work for the Harlin version.
- Morgan Creek gave Paul Schrader just $35,000 and a very short amount of time to finish his version of the prequel, and the meager outlay forced compromises in the film's post-production.
- By the time filming wrapped in February 2003, six writers had contributed to the screenplay and the budget had nearly doubled.
