Plot summary
After eight months as volunteer medics in a Peruvian civil war, where the wounded gave them subjects for the re-animation reagent, Herbert West and Dan Cain return to Arkham, Massachusetts. They take up their old hospital jobs, and West resumes work in the basement laboratory of Cain's house. Their materials come from the hospital morgue and a cemetery next door. West finds the reagent can wake single body parts, and he sets out to assemble a whole living woman from pieces of several. In the morgue he finds the heart of Meg Halsey, Cain's fiancée, and promises Cain it will bring a new Meg to life. Cain also treats Gloria, a dying patient, and keeps company with Francesca Danelli, a journalist he met in Peru. Lt. Leslie Chapham, investigating the Miskatonic Massacre, begins to ask the doctors questions.

- Jeffrey Combs was initially not going to return as Herbert West in Bride of Re-Animator because he was already booked to film The Pit and the Pendulum in Italy. When that production was pushed back on May 25, 1989, Combs was immediately cast to return as West.
- Barbara Crampton, who starred in the first Re-Animator, did not return for Bride of Re-Animator: a 1991 Fangoria report blamed soap opera obligations, but Crampton later said her agent convinced her not to make a cameo, feeling it beneath her.
- For Francesca's reanimated dog Angel, Doublin created a puppet, and when the dog was not shown as a puppet it was played by dog actor Friday, who was cast after demonstrating she could walk with a prosthetic arm.
- The special effects for the eye/finger creature were made by Tony Doublin of Doublin FX using a rod puppet, a stop-motion puppet and a stunt hand.
- Director Yuzna considered Patricia Tallman for the role of the Bride before choosing Kinmont for the part on May 28, 1989.
- Production on Bride of Re-Animator was scheduled to begin on June 5, 1989, which left the filmmakers less than one month to finalize the script, hire the cast and crew and get the special effects underway.
