Plot summary
In 1860, Baron Frankenstein faces the guillotine for the murders committed by the creature he built. Three years on he works in Karlsbrück as Doctor Stein, treating rich and poor alike. The town's medical council asks him to join it or stop practicing; he refuses, saying the council exists to eliminate competition. Doctor Hans Kleve, a junior councilor, waits in Stein's office and says he knows him as Frankenstein, offering silence in exchange for an apprenticeship. Stein takes him to the laboratory, where severed parts taken from patients react to a flame, a chimpanzee carries an orangutan's brain, and a body assembled from spare parts has no living brain. Karl, the deformed assistant who helped the Baron escape, has agreed to donate his brain for that body.

- Hammer's James Carreras presold the sequel in America, carrying a poster with him, then told writer Jimmy Sangster he had six weeks to write it before shooting began. Sangster objected that he had killed Baron Frankenstein in the first film, and Carreras replied that he would think of something.
- While The Revenge of Frankenstein was being made, Hammer was also producing a pilot in Hollywood for a planned American TV series, Tales of Frankenstein, which starred Anton Diffring.
