Can contain spoilers, watch the movie first.
- The film was originally written with Al Pacino as the criminal Rome and Stone Cold Steve Austin as the hero. After Austin and WWE parted ways in 2004 Randy Orton was set to star, but he turned the role down because of his bad conduct discharge from the Marines in the late 1990s, and was replaced by John Cena.
- Pacino turned down the role of Rome because of the low salary offered; Ray Liotta was then considered for it before Robert Patrick got the part instead.
- Principal photography was finished in 2004, two years before the film came out. To cover John Cena's absence from WWE events, a storyline was written in which Carlito beat him for the WWE United States Championship and Carlito's bodyguard, Jesús, stabbed him, so that he was taking time off to recover.
