Plot summary
Ogden Marsh, Iowa, is a small town where Sheriff David Dutten lives with his wife Judy, the community doctor. A resident walks onto the baseball field carrying a shotgun, and David is forced to shoot him. Judy notices another resident, Bill, speaking in lifeless, repetitive phrases. That night Bill seals his family in the farmhouse and sets it on fire. Three hunters find a dead pilot in a nearby bog, and David and his deputy Russell discover a military aircraft in the water there.

- The Machinist director Brad Anderson was hired to direct the remake but left the project after creative differences with the studio; he and writer Scott Kosar had aimed to make a film that did not feel derivative of the 1973 original or of 28 Days Later.
- The makeup crew based the infected's look mainly on rabies, tetanus and Stevens-Johnson syndrome, and lead make-up artist Rob Hall said the aim was to make the infection feel real, like something a viewer could catch.
- Director Breck Eisner tired of the cliché zombie look and chose a realistic design in which blood vessels appear to be bursting forth and face and neck muscles and tendons look tight; he called that look "hyper alive".
- Contact lenses worn by the actors covered their entire eyes and required eye-drops every five minutes to reduce injuries.
