Plot summary
Both men are told their lung cancer is terminal. Carter Chambers, an automotive mechanic and amateur historian, has been married to Virginia for forty-five years and has three grown children. Edward Cole owns the hospital where they are treated, has been divorced four times, and keeps a valet, Matthew, whom he deliberately calls Thomas. Edward's rule against private rooms puts them in the same room. The two find common ground, and Carter begins a list of what he wants to do before he dies, then throws it away. Edward retrieves it the next morning, adds items of his own, and offers to cover every expense. Virginia objects, but Carter joins Edward on the trip, with Matthew along.

- Justin Zackham coined the expression "bucket list" after writing his own "List of Things to do Before I Kick the Bucket" and shortening the title. The first item on it was to "get a film made at a major studio", and the list gave him the idea for this screenplay.
- Chicago Sun-Times critic Roger Ebert, who was diagnosed with thyroid cancer in 2002 and whose lower jaw was removed in 2006, gave the film one star, writing that it "thinks dying of cancer is a laff riot followed by a dime-store epiphany".
- Composer Marc Shaiman slipped a joke onto the score album: a rearranged version of the James Bond theme "Goldfinger", titled "Printmaster", sung in his own voice with lyrics spoofing the industry's habit of tracking music into scenes where it does not belong.
- The theme song, John Mayer's "Say", was left off the film's own soundtrack album and was included instead on the re-release of Mayer's third album Continuum.
- The film never had a weekend-to-weekend decline of more than 40%, closing on June 5, 2008 with $93,466,502 in the United States and Canada and $81,906,000 overseas, a worldwide total of $175,372,502 against a $45 million budget.
