Plot summary
Detectives hunting the killer known as Jigsaw find a fresh game scene while John Kramer stays hidden, bedridden with terminal cancer. His apprentice Amanda abducts Dr. Lynn Denlon from her hospital and fits her with a collar wired to John's heart monitor: it detonates if she moves out of range or if he dies. Lynn must keep him alive until another subject finishes his game. That subject is Jeff, a father consumed by vengeance since a drunk driver killed his son. At an abandoned meatpacking plant, Jeff faces a series of tests built around the people involved in the case, each one testing whether he can forgive.

- Lionsgate marketing president Tim Palen had 1,000 posters printed with a small amount of Tobin Bell's blood mixed into the printing ink, checking first that they were 'dead serious' about using real blood. They sold for $20, and the first was auctioned off for the Red Cross.
- At United Kingdom screenings of Saw III, five people were reported to have fainted at separate cinemas, three of them at one cinema, and ambulances had to be called.
- Saw III had to be toned down seven times before the ratings board would give it an R rating, according to executive producer Daniel Heffner.
- The production borrowed the bathroom set that Scary Movie 4, which had parodied the Saw franchise, had used.
- Darren Lynn Bousman, James Wan and Leigh Whannell all turned down a third Saw film, but at a lunch on the day they heard producer Gregg Hoffman had died, the three decided to make Saw III in dedication to him.
- The pig carcasses in the film were made out of foam, rubber and latex, and the pig props had live, disinfected maggots attached to them with honey.
- Monica Potter, who played Alison, the wife of Dr. Gordon in the first Saw, was asked to return for Saw III but declined out of a lack of interest, feeling she had done 'plenty' with the first film.
- Almost all of the film's transitions from one place to another were shot for real rather than with digital effects; for the move from Troy's crime scene to Kerry in the bathtub, Dina Meyer had to run, strip and jump into the tub.
