Plot summary
Nicky Godalin, a small-time Philadelphia mobster, hides in a cheap hotel room with an ulcer and a growing fear that his boss, Dave Resnick, has put a contract on him. He telephones Mikey, a friend from childhood, who brings milk and antacid and tries to calm him. Mikey works out a way for Nicky to leave Philadelphia, but the plan stalls, and the two men drift through the night, from a bar to a club, a movie theater, the cemetery where Nicky's mother is buried, and the apartment of Nellie, a woman Nicky has been seeing. Tempers fray as the hours pass and old grievances resurface. Elsewhere in the city, a contract killer is hunting for Nicky.

- After Paramount took possession, May hid two important reels of footage in her husband's friend's garage in Connecticut; the company traced the reels there but had no legal jurisdiction to search a house outside the state of New York.
- The shoot lasted 110 days in both Philadelphia and Los Angeles and used up 1.5 million feet of film, almost three times as much as was shot for Gone with the Wind.
- Cassavetes and Falk had both left the set and the cameras kept rolling for several minutes. A new camera operator said “Cut!” and was rebuked by May for usurping a director's command; when he protested that the actors had left, she replied: “Yes, but they might come back”.
- By the time production wrapped the budget had grown to nearly $4.3 million, and because May missed the delivery date, litigation with Paramount began in 1975, with the studio gaining possession of the film and negating her final cut privilege.
- May originally cast Yablans, the studio's own president, as a gangster, but Charles Bluhdorn, the chairman of parent company Gulf+Western, was not amused and demanded that she recast the part.
