Plot summary
A voodoo priest named Houngan brings Matt Cordell back from the dead, and Cordell takes up his badge once more. Meanwhile, two reporters hoping for a big break witness a convenience store robbery, where Officer Katie Sullivan intervenes in a hostage standoff. She wounds the suspect, Frank Jessup, and finds that the clerk, Jessup's girlfriend, had let him in on purpose. Shots are exchanged: Katie is severely wounded, and the clerk is killed. She is taken to a hospital and declared comatose. Detective Sean McKinney, the officer on the case, learns that the reporters have put out a story claiming Katie used excessive force against an innocent victim, a version that could clear the injured Jessup.

- The original script of Maniac Cop III had a Black detective investigating murders in a Harlem hospital, but the Japanese pre-sale distributor refused a Black lead and asked for Robert Davi instead; director William Lustig pleaded against it, and Davi was cast.
- Larry Cohen refused to rewrite Maniac Cop III's script unless he was paid for the additional work, and when the producers declined to pay him, director William Lustig had to improvise and cut scenes from the script.
- After the cuts, Lustig had only about an hour of usable film, so the producers went out to shoot filler scenes; Lustig then walked away from Maniac Cop III, which is credited to the pseudonym Alan Smithee.
- The Motion Picture Association of America rated the slasher sequel Maniac Cop III NC-17, and that rating was upheld on appeal.
