Plot summary
Arthur Fleck is held at Arkham State Hospital while awaiting trial for crimes he committed two years earlier. His lawyer, Maryanne Stewart, plans to argue that a second personality, the Joker, is responsible. At a music therapy session Arthur meets Harleen Quinzel, a fellow patient who says she grew up in his neighborhood, that her father was abusive and died in a car crash, and that she set her parents' apartment building on fire. She admires the Joker. As the two grow close, they imagine their lives as a stage musical.

The ending of Joker: Folie à Deux, in which a young Arkham inmate carves a Glasgow smile into his own face, was originally written for the first film, but Christopher Nolan blocked it, ruling that only Heath Ledger's Joker in The Dark Knight (2008) could have one.- When Lady Gaga shot scenes with a crowd of extras demanding Joker's arrest outside the New York County Courthouse, some people nearby mistook the shoot for Donald Trump's arrest after his first indictment.
- Before Joker had even finished shooting, Joaquin Phoenix told director Todd Phillips he was not ready to leave Arthur Fleck behind, and a dream of his character singing onstage gave him the idea of possibly making a musical sequel.
- Rather than singing to a pre-recorded track, Phoenix and Gaga performed the musical numbers live, accompanied by an off-camera piano player who kept up with whatever tempo they set; Phillips had to sync the very different takes in the edit, an experience he called a nightmare.

- Phillips dropped a scene in which Gaga, as Lee, allegedly improvised a kiss with a female protester during Arthur's trial and left it as a deleted scene, once he realized the moment would need dialogue from the woman that would ruin its vibe.
