Plot summary
Long ago, the sorceress Vivienne Nimue, called the Blood Queen, unleashed a plague on England until King Arthur cut her apart with Excalibur and scattered the pieces of her body across the country. In the present day, Hellboy, a demon who works for the Bureau for Paranormal Research and Defense, kills a vampire in Tijuana and hears a dying prophecy that the end is coming. His adoptive father, Trevor Bruttenholm, sends him to Great Britain to help the Osiris Club hunt three giants. A pig-like fairy named Gruagach is told by the witch Baba Yaga that collecting Nimue's limbs will win him revenge on Hellboy.

- Ed Skrein was originally cast as B.P.R.D. agent Ben Daimio but withdrew after discovering the character was Japanese-American in the comics, so that an Asian actor could take the part; Daniel Dae Kim was cast a month later.
- Director Neil Marshall disowned the film, calling it "God awful" and saying he does not consider it part of his canon, and blamed the producers for interfering with and contradicting his directions during shooting and post-production.
- A report published during the film's opening weekend said producers Lawrence Gordon and Lloyd Levin had the original cinematographer, Sam McCurdy, a collaborator of director Neil Marshall, fired; insiders called it a message that Marshall was not in charge, while Levin's attorney called it a group decision.
- Guillermo del Toro was offered a producer credit on the reboot but turned it down, wanting instead to direct his own script for Hellboy III, and Ron Perlman refused to return as Hellboy without del Toro's involvement.
- Doug Jones, who had played Abe Sapien in the del Toro films, was offered a cameo in the reboot but could not take part because he was committed to Star Trek: Discovery.
- Hellboy creator Mike Mignola suggested David Harbour for the title role after having seen his work on Stranger Things.
