Plot summary
In 1979, Alice Gould de Almenara arrives at a psychiatric ward, left at the gate by a man. Dr. Ruipérez questions her, reading a letter from Dr. Donadío. Alice says she is a private investigator and that she pretended to be mentally ill so she could be admitted and look into the death of Damián, the son of Dr. Raimundo García del Olmo. She also says her husband, Heliodoro, had her confined to take her wealth. Dr. Castell shows her around the building, and Dr. Arellano leads the sessions.

- Adapting Torcuato Luca de Tena's 1979 novel, director Oriol Paulo and co-writer Guillem Clua swapped the book's insulin shock therapy for electroconvulsive therapy because insulin shock was considered cruel even when the novel was published.
- The psychiatric hospital was portrayed by two real buildings: the old Tabacalera in Tarragona, built in 1923, stood in for the outdoor scenes, and a former Mercedes car plant in Barcelona was used for the indoor ones.
