Plot summary
In 1991 a gang stages a daylight robbery to take a rare gem worth ten million dollars from a bank's safe deposit vault, but two of the crew turn on their leader, Patrick Koster, and make off with the stone. A decade afterward, on the day before Thanksgiving, child psychiatrist Nathan Conrad is asked by his friend Louis Sachs to examine Elisabeth Burrows, a young woman at a state sanatorium. Out of prison for two weeks, Patrick and his remaining men break into an apartment overlooking the home Nathan shares with his wife Aggie and their eight-year-old daughter Jessie. That evening Jessie is kidnapped, and the demand is a six-digit number that only Elisabeth can supply.

- Don't Say a Word was due in cinemas nearly three weeks after the September 11 attacks. The filmmakers thought about delaying the release but decided not to, and instead cut and replaced shots of the World Trade Center, including the opening shot, which now shows Brooklyn.
- An earlier version of the screenplay did not feature the investigation side-plot set around Detective Sandra Cassidy, a police storyline that is part of the released film.
