Plot summary
NATO operative Jacques Kristoff is called to duty by General Zakev and given a mission: find Galina Konstantin, who has stolen a valuable container from the Slovak government, and take her and the cargo to Munich by train. With help from Lars, he reaches Galina just as a Slovak military unit closes in, and after a chase the two get aboard. Kristoff's wife Madeline and children Bailey and Ethan are on the same train, having planned a birthday surprise, and Madeline takes Galina for a rival. In the engine compartment two men seize control and cut the passengers off from the outside. They answer to Mason Cole, who is also aboard and wants Galina handed over.

- Jean-Claude Van Damme broke his hand while shooting the film but finished that day's work and took only one day off for treatment, so the director had to film around his swollen hand in several later scenes.
- Derailed holds the shortest average shot length in cinema history, 1.63 seconds, or 1.53 seconds in some sources, per a 2004 article by film scholar Barry Salt, and Guinness still recognized the record as of 2023.
- Bulgaria's railway system was so small that only four miles of track, near Aldomirovtsi, could be made available to the production, forcing extensive green screen to vary backgrounds; the main unit used the real train for only two days.
- The train-on-train collision, the helicopter's crash into the mountain and the final bridge collapse were shot with miniature trains built at 1:87 and 1:22.5 scales under South African model maker Willie Botha, with the helicopter, backgrounds and explosions added by CGI.
- Laura Harring lost her fear of heights while filming the wire escape scene, and the most dangerous aerials her character performed were done by Leonsia Dokuzova, an internationally touring Bulgarian acrobat.
