Plot summary
At The Renfrew Center of Florida, a forty-bed facility in Coconut Creek, women being treated for anorexia nervosa, bulimia nervosa and related conditions follow a structured routine of therapy, group sessions, shared meals and weigh-ins. Shelly Guillory, twenty-five and a psychiatric nurse, arrives with a feeding tube placed in her stomach; she has lived with anorexia for six years, five of them fed through the tube. Her identical twin, Kelly, does not have an eating disorder. Polly Williams, at the center nine weeks, began counting calories as a child. Brittany Robinson, fifteen, has liver damage, a slowed pulse and thinning hair. Alisa Williams, a divorced mother of two, was put on a diet at seven and links her anorexia to it.

- Greenfield and her director of photography Amanda Micheli lived inside the Renfrew Center for six months and were given unrestricted access to staff meetings, therapy sessions, mealtimes and the daily weigh-ins that structure an inpatient's day.
- Greenfield first came to the Renfrew Center in 1997 on assignment for Time magazine, then returned to photograph young women for her book and exhibition Girl Culture. Only after repeated visits did the center trust her enough to allow filming, and the result was her directorial debut, produced with R.J. Cutler.
