Plot summary
From March to June 2020, a New York City hospital system fights the Covid-19 pandemic. The film stays with the people inside one of the city's hardest-hit medical centers: the staff treating the sick and the patients in their care. Their separate experiences together show how the city endured the worst pandemic in a century, and the film also examines the structural factors that shaped the initial outbreak and how those factors affect American society more broadly. The project was announced in April 2020, after a contact from an earlier film opened the way to a hospital in the city. Two-person crews wore the same protective equipment as the hospital staff, and the virus did not reach any of them while they were filming.

- Matthew Heineman won access to Long Island Jewish Medical Center after reaching out to a source from his earlier film Escape Fire: The Fight to Rescue American Healthcare. His two-person crews wore the same personal protective equipment as hospital staff, and no crew member contracted the virus during production.
