Plot summary
A serial killer who spent four years in a Puget Sound institution gets out and starts looking for a new family to join. He has a surgeon alter his appearance, and the operation is performed without anesthesia. He settles in Deer View, California, under the name Keith Grant, rents a cottage and takes a job at a nursery. There he meets Christine Davis, a divorced school principal, and her son Andy, who has used a wheelchair since an accident; doctors think his inability to walk is psychosomatic. Andy, who is skilled with computers, is wary of Keith. Keith and Christine start dating. A former boyfriend of Christine's who has been watching them comes to the house to warn Keith away.

- For the scene in which Jerry Blake is given a new face, director Guy Magar filmed an actual plastic surgery procedure rather than using special effects.
- Terry O'Quinn reportedly declined to return as the killer in Stepfather III because he did not wish to be typecast as a homicidal maniac, so Guy Magar was approached by Chris Gorog to develop a premise that accounted for replacing him.
- To explain the recasting, Magar came up with a prologue in which The Stepfather gets plastic surgery, and Robert Wightman was cast in the role because he was the one best able to mimic O'Quinn's mannerisms.
- Because there was no convincing way to explain how The Stepfather survived his death in Stepfather II, Magar simply chose to ignore it.
