Plot summary
Anna Madden is eleven years old and ill with glandular fever. She draws a house, and when she sleeps she finds herself inside it. When she adds a face at the window, her next dream brings Marc, a boy with muscular dystrophy, who lives in the house. Her doctor tells her that Marc is a real person. She goes back into the house in her dreams many times. Each new detail she puts into the drawing turns up there as well: stairs so she can climb up to Marc, more items for his room, a fruit tree in the front yard. In waking life her lying strains her relationship with her mother, and her father is often away for work and drinks heavily.

- Bernard Rose read Catherine Storr's novel Marianne Dreams on the advice of his ex-wife, and replaced the book's threat of abstract phallic-shaped stones with a man with a hammer, which he felt was psychologically the same but made the film heavier and more adult.
- After failing to find financial backing for Paperhouse in his native Britain, Bernard Rose found support from Vestron Pictures, which backed the film only on the condition that it would not receive an R-rating.
