Plot summary
Madampalli Tharavad, the ancestral mansion in Nakulan's village in Kerala, is widely believed to be haunted. He brings his wife Ganga there over the objections of his uncle Thampi, and relatives join them at the estate. The family warns the couple away from the Thekkini, a locked southern wing. Legend holds that 150 years earlier the patriarch, Karanavar Sankaran Thambi, brought a court dancer named Nagavalli back from Thanjavur, and that her spirit stayed shut inside the room. Ganga takes the warnings for superstition and has the wing opened. Apparitions appear, objects shatter, and her saree catches fire. When purification rituals fail, Nakulan sends for his friend Sunny Joseph, a psychiatrist working in the United States.

- Manichitrathazhu was inspired by a real-life tragedy in the Alummoottil family at Muttom near Haripad, in central Travancore: the family's inheritor and his domestic worker were murdered by the son-in-law after he was written out of the marumakkathayam inheritance system.
- Nagavalli's Tamil lines were dubbed by Tamil artiste Durga, hired by Fazil after crew including editor Shekar felt Bhagyalakshmi's voices for Ganga and Nagavalli sounded too similar; Fazil never credited Durga and forgot to tell Bhagyalakshmi, who was unaware of it for a long time.
- Writer Madhu Muttam is credited as the sole writer of the four remakes only after he filed a copyright suit against Fazil, who had begun being listed as the writer of the original script in the remakes.
- The film's haunted Thekkini room was built as a set inside the Vasan House in Chennai, the home of the late S. S. Vasan, the founder of Gemini Studios.
