Plot summary
Paul Toombes made his name playing Dr. Death, a skull-faced killer, in horror films. At a Hollywood party for the fifth, he tells the guests he is to marry Ellen Mason, who hands him an engraved watch. Oliver Quayle, a producer of adult films, then informs Paul that Ellen had appeared in his pictures and had been his lover. Paul's angry outburst drives Ellen to her room, where a masked man dressed as Dr. Death comes at her with a knife. Paul is acquitted of the killing, yet he cannot tell whether he was responsible. His career gone, he spends years in a mental hospital. Twelve years on, his friend Herbert Flay, a screenwriter, brings him to London for a Dr. Death television series, and on the crossing he meets Elizabeth Peters, a young actress who pursues him.

- Madhouse gives "special participation" billing to Basil Rathbone and Boris Karloff, who had died in 1967 and 1969. Their scenes were lifted from earlier AIP films they had made with Vincent Price: Rathbone's from Tales of Terror, Karloff's from The Raven.
- Director Jim Clark called Madhouse "stillborn" in his autobiography and said Milton Subotsky's edits were the final nail in its coffin. Its script, by Ken Levison, was still being rewritten during production because of its quality.
- Madhouse was the last film Vincent Price made for American International Pictures, the studio behind many of his horror movies, as his contract was not renewed.
