Plot summary
Nightclub owner J.P. Monroe buys a carved pillar that holds Pinhead, a Cenobite separated from his human half. Television reporter Joey Summerskill witnesses hooked chains tear apart a young clubgoer in a hospital emergency room. A homeless woman named Terri says the chains came from a puzzle box she pulled out of the pillar and gives the box to Joey. Joey begins looking into the box's history with her cameraman, Doc, and invites Terri to stay at her home. Pinhead lies dormant until Monroe's encounter with a clubgoer awakens him. Monroe then promises to supply Pinhead with more clubgoers in exchange for power.

- 20th Century Fox demanded extensive cuts to Clive Barker's Nightbreed and marketed it misleadingly as a slasher film; it underperformed and put the production company Film Futures out of business, which, with New World's own bankruptcy troubles, left Hellraiser III in development hell.
- Hellraiser III's score was originally meant to be rock music, but it went over poorly in test screenings, so Randy Miller wrote a more traditional orchestral score in three weeks; on a restricted budget, it was performed by the Mosfilm State Orchestra and Choir in Russia.
- According to Anthony Hickox, Bob Weinstein loved his rough cut and offered money for extra effects work and a new ending, which led to the early use of CGI, the first in a horror film, for several gory scenes.
- Randel was attached to direct Hellraiser III but was removed by the producers, who worried his vision for the film was too bleak; Anthony Hickox replaced him after a mutual friend Hickox had cast in Waxwork II: Lost in Time suggested him.
- The Black Mass scene caused controversy in socially conservative North Carolina; refused permission to shoot inside a real church, Anthony Hickox used a matte painting behind the altar, and when crew members complained of sacrilege he compared it to Hammer horror films in which Christopher Lee rampaged through churches as Dracula.
- The character Barbie's flamethrowers were made by putting gas and a 40-foot-long flamethrower into a fiberglass head, with the gas building up inside a fake torso before the fire blasted through the top head.
- Terry Farrell, who was returning to acting after a break, was the first actor cast; Anthony Hickox had wanted her for a role in Waxwork II, but a scheduling clash made that impossible, so he promised to cast her in his next project.
- Clive Barker originally intended the Hellraiser sequels to focus on Clare Higgins' character Julia Cotton, but Higgins declined to return for a third film, and the character was killed off in later drafts of Hellbound.
