Plot summary
Billy Lenz is born with severe jaundice from a liver disease, and his mother, Constance, abuses him. Constance and her lover kill Billy's father, Frank, on Christmas Eve and bury him in the crawlspace, then lock Billy in the attic, where he stays until he escapes on Christmas Day 1991. Police send Billy to a mental asylum, and his young sister, Agnes, goes to an orphanage. Fifteen years later, on Christmas Eve, Billy escapes his cell and returns to the family house, which is now a sorority house at Clement University in New Hampshire.

Glen Morgan's original script ended with Kelli and Leigh in the hospital receiving a phone call from Billy, but Bob Weinstein scrapped it and asked for a new ending; the theatrical cut closes with Billy impaled on the hospital's Christmas tree-topper.- Christian groups including Liberty Counsel and Operation "Just Say Merry Christmas" called the film "offensive", "ill-founded" and "insensitive" over Dimension's decision to release it on Christmas Day.
- Glen Morgan based the killer Billy Lenz on Edmund Kemper, a real-life serial killer who as a child was locked in his home's basement by his mother, whom he later murdered.
- On her first day of shooting, during a fight scene with the killer, Lacey Chabert injured her leg and had to fly to Los Angeles to see a doctor, but she returned to the set the following day and kept filming.
- Michelle Trachtenberg had wanted to avoid returning to horror unless she was "given the opportunity to die" onscreen; reading the script alone at night unnerved her, which influenced her decision to sign on as Melissa Kitt.
- Promotional footage shot for the film, including Michelle Trachtenberg aiming a shotgun and saying "Merry Christmas, motherfucker" and Lacey Chabert being dragged through the snow, was never incorporated into the film but did appear in the official theatrical trailer and television spots.
- Because much of the film takes place during a blackout inside a home, the cinematographers and lighting department cut holes in the walls to get appropriate lighting on the actors.
