Plot summary
At St. Mary's Medical College in Mangalore, Arjun Reddy Deshmukh is a house surgeon whose academic excellence is undercut by anger he cannot control, giving him a fearsome reputation on campus. An inter-college football match turns into a violent brawl, and the dean demands a public apology or expulsion. Arjun chooses to withdraw, then reverses that decision after meeting Preethi Shetty, an introverted first-year student. He manages her academic schedule and shields her from harassment on campus, and she gradually returns his affection. They stay together through his graduation and his move to Mussoorie for a master's degree in orthopedic surgery. When he visits Preethi's home to seek her family's approval to marry, her father Devdas Shetty refuses the alliance, and Arjun's grandmother counsels patience.

- Sandeep Reddy Vanga first approached Sharwanand for the lead role in Arjun Reddy; Sharwanand was at first wary of Vanga handling directing and producing at once but changed his mind after reading the script. He then left the project, and Vanga cast Vijay Deverakonda as Arjun Reddy.

- Arjun Reddy was partly inspired by Vanga's own life as a physiotherapy student: he said it was not his story but had many references from his life, and some of his medical college friends found Arjun reminded them of him.
