Plot summary
Behind the Bodega Bay Inn, in the cemetery at Shady Oaks, Pinhead opens André Toulon's casket and pours a vial of liquid over the corpse, and the skeleton's arms rise. Months later, parapsychologists Carolyn Bramwell and her brother Patrick arrive at the hotel with Lance and Wanda to investigate the death of Megan Gallagher, whose brain was taken. Alex Whitaker, driven insane by earlier events, is held in an asylum and suspected of the killing. The group's guest psychic, Camille Kenney, warns them that the hotel is not safe, and Camille's son Michael arrives worried about his mother. Among the puppets is a new member, Torch, whose arm holds a flame-thrower.

- Director Dave Allen was so difficult on set, taking hours to produce very little work and treating the puppets better than the actors, that producer David DeCoteau wanted to fire him, but Charles Band refused because Allen owned the puppets and Band needed them for sequels. The film went five days over schedule with 63 incomplete scenes.
- Paramount Pictures, Full Moon's distributor, found the Leech Woman puppet "disgusting" and wanted her killed off, though crew members said Charles Band chose to kill her because he thought she did not work as a character. Writer David Pabian wrote the scene in which Martha kills Leech Woman.
- The Tunneler puppet's eyes were closed the entire time, so the effects team, according to Chris Endicott, had to put black tape on them in some scenes to make them appear open.
- Steve Welles spent hours in makeup and head-wrapping in bandages to play the undead André Toulon, a look inspired by The Invisible Man and The Phantom of the Opera. Makeup artist David P. Barton used a photograph of the Borremose man as reference for Toulon's decomposed face.
- Carolyn Bramwell and Elsa Toulon, both played by Elizabeth Maclellan in Puppet Master II, were her last acting roles.
- Charlie Spradling, who played Wanda, later became the host of the Full Moon Fan Club and official spokesperson for Full Moon.
