Plot summary
As a computer virus based on Pac-Man invades Japan, a group of doctors attack Genm Corp and begin a wave of terror using stolen Proto Gashats. Ghost and Ex-Aid find themselves at the center of the fight in an effort to protect those most important to them. But they aren’t alone, as Heisei Riders from the past join them to stop Dr. Pac-Man!

- Because Kamen Rider Ex-Aid is themed on games, producer Omori proposed a tie-in with a major game character and Pac-Man was brought into the film. Since Pac-Man himself is not a villain, he could not be used as the boss and was instead built up as the mid-film climax.
- According to director Koichi Sakamoto, Gaku Sano, who plays Kouta Kazuraba / Kamen Rider Gaim, could not appear because of his schedule, but his voice was used with his consent. An assistant producer rewatched every episode of Gaim and noted fitting lines, and the sound team cut out and inserted the audio in repeated trial and error.
- On 26 December 2016 the first cheer-screening event in Kamen Rider history was held at Marunouchi TOEI, with Hiroshi Fujioka, star of the co-screened Kamen Rider 1, and Shun Nishime giving stage greetings.
- Chihiro Yamamoto was cast because she had trained at the gym of Alpha Stunt, the stunt agency that director Koichi Sakamoto belonged to, and she put her Chinese martial arts skills to use in the film's fight scenes.
- Ryoma Takeuchi, by then already a popular actor, did appear as Shinnosuke Tomari / Kamen Rider Drive, and from the next film onward it became customary for former Kamen Rider actors who had grown famous to appear again.
