Plot summary
Jay Height is a college student. At a movie with her boyfriend Hugh, she watches him grow uneasy over a girl in a yellow dress that she cannot see, and he wants them to leave. Later the two sleep together for the first time in his car, and Hugh renders Jay unconscious with chloroform. She comes to in a wheelchair, bound, and he tells her that the encounter gave her something: an entity that only Jay and those it has afflicted before can see, able to take the appearance of any person and moving toward her at a steady pace while always aware of where she is. If it catches her, it kills her. A naked woman walks toward them to prove the claim, and Hugh urges Jay to sleep with someone else soon, drives her home and flees.

- The filmmakers had to abandon their planned alternate location for the scene where Hugh drugs Jay before showing her the entity, because a murder there had turned it into an active crime scene. The scene's exteriors were shot at an abandoned psychiatric hospital and its interiors at the abandoned Packard Automotive Plant.
- Writer-director David Robert Mitchell said the idea came from recurring dreams he had in his youth about being followed, which he linked to anxiety over his parents' divorce when he was that age.
- Cinematographer Mike Gioulakis said he and Mitchell were both fans of still photographer Gregory Crewdson, whose surreal suburban imagery shaped the film's monster, shot composition and overall look.
