Plot summary
Simon Helder, a young doctor, is arrested and accused of sorcery after a captured body snatcher gives his name to the police. He is sent to an insane asylum as an inmate. There he recognizes the institution's surgeon, who calls himself Dr. Carl Victor, as Baron Victor Frankenstein, the man whose experiments he had studied and tried to copy. Frankenstein is presumed dead, yet he lives and works in the asylum, holding incriminating evidence against Adolf Klauss, its corrupt director. He takes Simon on as an apprentice, and a girl named Sarah, nicknamed Angel, assists him. The two doctors set to work designing a new creature.

- Peter Cushing was 59 and in poor health when he insisted on doing his own stunt for Frankenstein and the Monster from Hell: he leapt from a tabletop onto David Prowse's back and spun in circles to sedate the runaway creature.
- David Prowse played Frankenstein's creation for Hammer twice, in The Horror of Frankenstein and then in Frankenstein and the Monster from Hell, and is the only actor to have played a Hammer Frankenstein's monster more than once.
- Peter Cushing helped design the wig he wore as Baron Frankenstein in Frankenstein and the Monster from Hell, then regretted it years later and joked that it made him look like the American star Helen Hayes.
- Frankenstein and the Monster from Hell was the sixth and last time Peter Cushing played Baron Victor Frankenstein, a part he originated in Hammer's 1957 film The Curse of Frankenstein.
- David Prowse said on the DVD commentary that turning into the Monster from Hell took him only about 30 minutes, suiting up and pulling on the mask, while his make-up for an earlier Hammer monster had taken several tedious hours.
