Plot summary
At the Caligari Insane Asylum, Dr. Caligari moves glandular brain fluid from one patient into another, in an attempt to cure them, and her subjects have not agreed to it. She is the granddaughter of the original Dr. Caligari. Gus Pratt, a serial killer who eats his victims, is among them, along with Mrs. Van Houten, a housewife whose husband has been told that her trouble is a disease of the libido. The fluid carries conditions and traits from person to person. Mr. and Mrs. Lodger, a married pair of doctors, grow worried about the experiments and take their concerns to Mrs. Lodger's father, Dr. Avol.

- Sayadian made the modern update his own by giving the 1989 film overt S&M overtones and changing Dr. Caligari from a little old man into an intense female dominatrix, while keeping the expressionist style and interiors-only approach.
- Sayadian shot the film during the 1988 Writers Guild of America strike, using only interiors, and the work stoppage let the production use Los Angeles production resources that would otherwise have been unavailable.
