Can contain spoilers, watch the movie first.
- On 16 May 1989, after two months of filming, an SA 360 Dauphin helicopter took off to shoot a scene, veered left and plunged into a 40-foot ravine, killing electricians Don Marshall and Mike Graham, cameraman Gaddi Danzig and pilot Jojo Imperial. Actor Geoff Brewer later died of his injuries, bringing the toll to five.
- Costume designer Katherine Dover, who witnessed the accident, said smoke became so thick it was hard to see the wreckage and that stunt performers ran to the site calling for water and fire extinguishers, but neither was available; the two vehicles designated as ambulances were empty vans containing mattresses, with no plasma, oxygen or bandages.
- According to one account of the film's helicopter accident, the pilot had raised concerns about the helicopter's maintenance and proposed postponing filming until the following day, but Aaron Norris became angry at the suggestion and threatened to replace him if he did not proceed. Filming went ahead as planned.
- This was the second helicopter accident on a Norris film. The first happened during the filming of Missing in Action III in the Philippines, when four soldiers working as extras were killed in the crash of a military helicopter rented by the film company.
- Chuck Norris said in 1990 that the abandoned hilltop mansion had never been used and was left an empty shell when Marcos was booted out: the production bought it, made $1 million worth of refurbishments, even built a swimming pool, and then blew it up.
- The unfinished hilltop mansion in the mountain resort of Tagaytay was called the People's Park in the Sky. Imelda Marcos had started building it in 1983 as a guest house for a visit by U.S. President Ronald Reagan that never took place.
- Jean-Claude Van Damme was offered a role in the film but declined it in order to star in Cyborg. He later recalled training with Norris three times a week and being employed by Norris during his early years in the United States.
