Plot summary
In Acre, Israel, archaeologist Carl Bugenhagen tells his friend Michael Morgan that Damien Thorn is the Antichrist, and shows him a mural in which the boy's face appears. Seven years later, Damien lives near Chicago with his uncle Richard Thorn, who heads the family business, and his step-aunt Ann, along with their son Mark; the two boys are enrolled together at a military academy. Richard's aunt Marion dislikes Damien and warns Richard that unless he separates the two boys, she will leave him out of her will. At the academy, Sergeant Neff singles Damien out for attention, and one of the teachers is startled by the boy's grasp of historical dates.

- Original director Mike Hodges said producer Harvey Bernhard pulled a gun on him during an argument over the design budget; Hodges insisted he quit on his own rather than being fired, contradicting the studio's official story of 'artistic differences'.
- William Holden turned down the lead role in the first Omen because he did not want to appear in a film about the devil, and Gregory Peck took the part instead; once that film became a hit, Holden made sure not to pass up the sequel.
- Leo McKern, who plays archaeologist Carl Bugenhagen, is the only actor in the Omen film series to play the same character across more than one installment; actor Tommy Duggan appears in two films but as different characters each time.
- Jerry Goldsmith's Omen II score was recorded in the US, but the soundtrack album had to be re-recorded entirely in Britain because a since-abolished musicians' union rule would have doubled the orchestra's fee had the same recording been used for both the film and the album.
- Scenes meant to be set in a New York rail yard were actually shot at a Chicago freight yard with the city's CBOT Tower and Sears Tower visible in the background, and the Thorn Industries headquarters was really Chicago's City Hall.
