Plot summary
Psychiatrist Dr. Dorothy Otnow Lewis is fascinated by what the human brain can do when it is ruthless. She spends her career studying the inner lives of violent people, including murderers. With each case, she moves closer to a unified theory of what makes a killer. Her approach departs from the views of many colleagues. She also explores multiple personality disorder.

- Crazy, Not Insane grew out of a planned miniseries Alex Gibney was making with Laura Dern as a psychiatrist who saw death-row patients. Psychiatrist Dorothy Otnow Lewis agreed to consult on it and showed Gibney videotapes of her past cases.
- The documentary was first called Dorothy and Ted and was to follow Dorothy Otnow Lewis investigating serial killer Ted Bundy and reflecting on her 1989 interview with him, but Bundy ended up with only a smaller part in the final cut.
