Coffy is an emergency room nurse in Los Angeles whose younger sister Lubelle was hospitalized after taking contaminated heroin. Posing as a prostitute desperate for a fix, she gains entry to the homes of a drug pusher and a mob boss and kills them. She goes back to her hospital shifts, where her police friend Carter, an officer who refuses to bend the law for the mob, offers her a ride home. Two hooded men break into Carter's house and beat him, and Coffy takes up her vigilante work again. Her boyfriend Howard Brunswick, a city councilman, announces a run for Congress and the purchase of a nightclub. Her next targets are the pimp King George and the Mafia don Arturo Vitroni.

Can contain spoilers, watch the movie first.
- The film exists because AIP's head of production Larry Gordon lost the rights to Cleopatra Jones after a handshake deal; he asked Hill to quickly make a revenge picture and beat it to market, Hill brought in Pam Grier, and the result earned more money than Cleopatra Jones and established Grier as an icon of the genre.
- Coffy is a favorite of Quentin Tarantino, who ranks it high among his top 20 best films; he later hired Grier for Jackie Brown in 1997, a film with clear inspiration from films like Coffy and Foxy Brown.
- A sequel called Burn Coffy Burn was planned, but because sequels were performing poorly at the box office at the time the plans were scrapped and replaced with Foxy Brown.
