Plot summary
Someone comes out of the sea and sets upon two visitors at a beach on a remote Greek island. In Athens, five travelers prepare to tour the islands, and Julie joins them, asking for a ride to the island where friends of hers live. Carol alone objects, her tarot cards having convinced her that something bad will come of the trip. While disembarking, Maggie hurts her ankle and stays aboard. A man attacks the boat. The others find the island's town in disarray and abandoned, except for a woman in black who writes "Go Away" on a dusty window.

SPOILER
The scene in which the titular man eater strangles a pregnant woman, tears out the fetus from her womb and bites into it made the film one of the infamous "video nasties" prosecuted in the United Kingdom in the early 1980s, and the controversy greatly aided its cult reputation.SPOILER
D'Amato said he had to shorten the scene in which Montefiori devours Serena Grandi's fetus, which was in actual fact a skinned rabbit covered in blood.- The score was performed by Marcello Giombini on two ARP 2600 synthesizers; the original recordings are considered lost, and the U.S. release, The Grim Reaper, used music from Kingdom of the Spiders instead of Giombini's score.
- Many of the film's interior shots were made in a small villa in Sacrofano owned by Donatella Donati's father, Ermanno Donati, which had already been used as a set for some Black Emanuelle films; the "Greek island" the boat approaches is really Ponza, in Italy.
