Plot summary
Terry, Nancy, and Gloria are on their way to a music festival in rural Washington when a storm makes Nancy lose control of the car. Terry wakes in a secluded mansion owned by the Penroses. Marion, a middle-aged spinster, lives there with her elderly, crippled mother and a groundskeeper named Norman. Because Gloria is the one who was badly hurt, Marion proposes that the three women spend the night. Over dinner Mrs. Penrose, who is senile, delivers tirades against men and keeps accusing Marion of letting men into the house. A strange man peers at the women through the windows. Terry finds a human tooth beneath her bed and hears someone breathing heavily upstairs, and while she and Nancy shower, an unseen person watches through a peephole in the wall.

- The BBFC passed Unhinged uncut for UK cinemas in 1983, but the DPP retroactively banned its video release, placing it among 72 "video nasties" for violating the Obscene Publications Act. Unlike other titles on the list, its murders were only suggested, not shown.
- Unhinged was one of several films whose videos the British government confiscated in police raids beginning in December 1983, alongside The Boogeyman, The Driller Killer and The Last House on the Left.
- Because Pittock Mansion in Portland is a tourist attraction and city property, most of Unhinged had to be shot overnight, roughly 5 p.m. to 7 a.m., over nineteen evenings, with dinner breaks at 10 p.m.
- Richard Blakeslee wrote in a newsletter during the shoot that Don Gronquist was "going a little crazy" and kept losing pages of his script, which turned up splattered with stage blood, misplaced, soaked with water and covered in muddy footprints.
- Don Gronquist said he and Reagan Ramsey devised the story while drinking together in local bars, wondering how cheaply they could make a commercially viable 35 mm film.
