Plot summary
Kōryū Li travels to Yokohama after her brother Mansei, a shorinji kempo champion who works as a Hong Kong drug agent, disappears while investigating a company called Central Export. She visits her uncle's restaurant and her cousins Jirō and Reiko, then takes up his last lead at Club Mandarin. A red rose delivered there is the signal to seek out Mansei's partner Fanshin, a stripper at the club. Assassins for Central Export move to seize Fanshin, and Kōryū fights them on her own. At the shorinji kempo school where her brother trained, under Tetsudo Fujita, she is introduced to the student Seiichi and to Emi Hayakawa, while Central Export brings in the mercenary Kazunao Inubashiri to watch the school.

- After The Street Fighter was a hit, Sister Street Fighter was rushed into production with Etsuko Shihomi in the lead; she belonged to Sonny Chiba's Japan Action Club and had only played a supporting role in the earlier film.
- New Line Cinema's American version was first rated X for violence by the MPAA, after which the distributor cut roughly 6 minutes of graphic footage, removing all shots with considerable amounts of blood and gore.
